Long Term Conditions Service

We are fortunate to have a fantastic team of long term condition nurses. If you have one of the following conditions you will be part of our annual recall system:

  • Diabetes
  • Asthma
  • COPD
  • Chronic kidney disease
  • Heart disease
  • Previous stroke
  • Peripheral vascular disease
  • High blood pressure

Smoking Cessations

If you have been advised by the surgery to submit a smoking review on a regular basis please use our online Smoking Review form.

INR Clinics

Our nurses run INR testing and dosing clinics at the surgery. Please make an appointment.

Cervical Smears

You will receive a notification to book a smear from NHS screening. Please book an appointment with our nurses. It is important you let reception know the reason for your appointment so the correct paperwork, appointment length and nurse can be arranged for you.

Family Planning

For family planning queries please make an appointment or ask a nurse.

Antenatal Clinic

All antenatal checks are now done by the community midwife, unless you are higher risk and seen at the hospital.

Midwife Appointments

These clinics are led by our Community Midwifery team.

Children and Young People

We offer an 8 week check to all new mums and their baby (or babies). This is a double appointment with the GP and a nurse appointment soon after for the first set of immunisations.

The health visiting and school nursing teams are not based in our building but we hold regular team meetings with them to co-ordinate any shared care.

Teen Health

We encourage teens to share their health care needs with their parent or guardian but are happy to see teens confidentially who access our services independently.

Our receptionists are specifically trained to offer appropriate services when you call up or drop in.
