NHS Health Checks

We will be writing to our patients aged 50 - 74 who do not have existing long term conditions such as heart disease, diabetes, hypertension, stroke or kidney disease, on a rolling five year programme inviting them for a NHS Health Check. The check is to assess a person’s risk of developing these medical conditions and to offer advice on how to reduce the risk and stay healthy. Please take up this invitation when you receive it. The check will include taking your family history, blood pressure and a simple blood test to check your cholesterol level. We are inviting patients on the basis of information we already hold or need to gather so do not worry if you do not receive your invitation for some time. If you are concerned about your own level of risk, please contact the surgery to request a Health Check at any time before you receive your invitation.

We want to make it easy for our patients to order repeat medication, and you can request your medication by fax or online, by post, in person or via a pharmacy.

Your request will be processed within TWO working days of receipt so please allow sufficient time to order your medicines before you run out. Prescriptions will be ready after 2pm each day.

We regret that  we cannot accept telephone requests for repeat prescriptions.  This is to remove the possibility of mistakes being made in transcribing telephone requests and also to free up our telephone lines for patients urgent medical needs. 

IN PERSON: It will help us to provide a better service if you return the white, right hand portion of your previous prescription, indicating which items you are requesting. If you cannot do this, please complete one of the yellow request forms, which are on the reception desk.

POST: Please type, or write clearly your name, address, date of birth, plus the name of your medications, the strength and the quantity – for example, patient number 001, Aspirin 75mg x 28.

To maintain confidentiality if you use fax or e-mail to request your prescription, you need only provide your patient record number (shown on the top right hand side of your last prescription) plus to name of your medication, the strength and the quantity – for example, patient number 001, Aspirin 75mg x 28.

Collection of Prescriptions

Most pharmacies offer a service to collect prescriptions from the surgery on behalf of patients. The pharmacies then get the medicines ready for patients to collect direct from the pharmacy, which saves patients a trip to the surgery.
