Patient Registration
Anyone who lives within our practice boundary can register as one of our patients. If you are unsure if you live within our boundary, there is a map of the practice area in the waiting room that you can view, or please ask for advice from our receptionists.
If you wish to register, please see one of our receptionists who will give you a simple form to complete, or complete the pre-registration form (link below). It will be helpful if you can include details of your previous or current GP, and your NHS number.
Pre-registration Form
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
